Welcome to the San Mateo County Medical Marijuana Identification Card Program. The program is located within the Office of Vital Statistics located in the San Mateo County Health Department at 225 37th Ave., San Mateo. San Mateo County residents may apply for cards in the Office of Vital Statistics during the following times (please note that the office is closed from noon-1pm):
Monday through Wednesday: 8:00 am-12 noon and 1:00 pm-5:00 pm
Thursday: 8:00 am-12 noon and 1:00 pm-4:00 pm
Friday: 8:00 am-12 noon and 1:00 pm-5:00 pm
The annual fee for a Medical Marijuana Identification Card is $98.00. Verified Medi-cal beneficiaries pay a reduced fee of $49.00. Once an application is submitted and the verification process begins, all payments are non-refundable. The Office of Vital Statistics may be reached at (650) 573-2395.
